Conclusions:
- ADEs and ADRs represent significant burden to the healthcare system
- Can account for 1-5% of hospital admissions overall, and up to 10% for elderly
- Can occur frequently during hospitalization
- Tend to occur more for inpatients (treated for more acute disease and have more concomitant medications)
- Medication errors can occur in many scenarios: prescribing, transitioning, dispensing, administration, and monitoring
